Confined space again

This is a doozy . A space that was safe, but turned into a confined space by isolating of fans and the spraying of chemicals. No testing before the second entry. Complacency kills. I have so many questions about this. I’m nowhere knowledgeable about biofuels but my understanding is they are difficult for ship use. It must be a blend?

The OBO Derbyshire was lost with all hands during a typhoon. 44 crew and 2 wives who were onboard. The ship was four years old and had an experienced Master. She was the last of a series of ships built and until her sister ships experienced cracking around frame 65 there was no formal investigation....

It would all go back to the original weld procedures and if they were followed. In Canada we have the CWB, the Canadian Welding Board, which regulates the industry, the training of apprentices,weld inspectors, standards and procedures. Then there is Class inspection, in this case LR, which may be so...

<facepalm> Exactly! access tanks, remove outfitting, weld grinding, reweld, retest, reinstall, recoat, heat affected zones. It is LR oversight, so there must have been QA involved. The ship was floated out months ago and sent to Victoria for outfitting.
