It is just painful to read about FV accidents year after year. TSB does their investigation and seems to be ignored, then the next accident occurs. Where was TC in all of this? I'm reading on Twitter that crew rotations are being affected and reliefs can't fly in to meet ships. As usual, its the ships crew that are the last to be thought about.
