We were anal about draining the lines on every ship I was on or running the system with the anchor wash open. Running was a drag because it took all the warm water out of the seabay.....those were the days that I don't miss...-28*C and counting. lol

Yeah, Normally on that ship we were too - full procedure for draining.
But having missed Christmas, late for crew change, and the deck hand on watch that was supposed to be doing the draining procedure was going home the next day... To put it simply, he didn't do as comprehensive a job as necessary. And it was about -55Deg C, didn't take much.

In my experience it was always, get off the ship and let local authorities deal with. Seems counter intuitive, but why risk life and limbs for a piece of metal.

Here's a pic of the Svitzer Bedford which was towing a barge up the St Lawrence, the famous hot turbo / oil line failure - engine room fire. This happens near Quebec City, the local Groupe Ocean tugs grabbed the barge, and the Bedford, brought it alongside to the Port of Quebec where a massive response by local firefighters was mounted.
